Supplemental Health Benefits
Round out your coverage with plans that pay you cash when something happens.
These three plans pay a benefit directly to you, on top of whatever your medical plan covers, and you can spend it however you like. You pay the premium through after-tax payroll deductions at the group rate we’ve negotiated — often lower than you’d pay as an individual. Coverage is available for your dependents as long as you cover yourself.
All three pay you for a check-up. Each plan includes an annual benefit just for completing preventive care or a health screening.
Accident Insurance
Accident Insurance helps cover the costs of unexpected bills due to covered accidents, regardless of any other insurance you have. If you’re hurt in a covered accident you receive a cash benefit you may spend as you like.
Use it to offset:
- Copays and coinsurance
- Home health care costs
- Everyday expenses such as utilities and groceries
Coverage Amount:
- Cash benefit based on type of accident (ranges from $75,000)
- Coverage is available for your dependents as long as you cover yourself
Examples of covered injuries: broken bones, burns, torn ligaments, cuts that require stitches, eye injuries, ruptured discs, concussions.
Once a year you can receive a $75 benefit for getting your preventive care or health screening.
Critical Illness Insurance
Critical Illness Insurance provides a cash benefit to help pay for medical expenses not covered by your medical plan, as well as day-to-day expenses that add up while you’re ill — rent, mortgage, car payments. If you’re diagnosed with a covered illness you receive a lump-sum cash benefit, even if you receive other insurance benefits.
| CRITICAL ILLNESS | COVERAGE AMOUNT |
|---|---|
| Employee | $10,000, $20,000 or $30,000 |
| Spouse / domestic partner | 100% of the employee amount |
| Child(ren) | 100% of the employee amount |
| Guaranteed issue | Up to $30,000 |
Examples of covered illnesses: cancer, heart attack, major organ failure, end-stage renal (kidney) failure, coronary artery disease, stroke.
Once a year you can receive a $50 benefit for getting your preventive care or health screening.
Hospital Indemnity Insurance
Hospital Indemnity Insurance provides a fixed lump-sum payment to help cover hospital expenses not covered by insurance, or to pay for expenses while you, your spouse or domestic partner, or your dependents are in the hospital.
| HOSPITAL INDEMNITY | BENEFIT |
|---|---|
| Hospital admission | $2,000 |
| Hospital stay | $200 per day, up to 30 days |
| Hospital ICU stay | $400 per day, up to 30 days |
| Hospital chronic condition admission | $50 |
| Hospital observation stay | $100 per day, up to 3 days |
| Newborn nursery care stay | $200 per day, up to 30 days |
Once a year you can receive a $50 benefit for getting your preventive care or health screening.
Important notice: this is a fixed indemnity policy, not health insurance. It may pay you a limited dollar amount if you’re sick or hospitalised. You’re still responsible for the cost of your care. This policy is not a substitute for comprehensive health insurance.
