Eligibility

Who can enroll, and who you can cover.

REMINDER:

You have 31 days from your hire date (or from the date of a qualifying life event) to enroll or make changes. If you miss the window, you will have to wait until the next annual enrollment.

Who is Eligible

You are eligible if you work at least 30 hours per week. Most benefits are effective on your date of hire, as long as you enroll within 31 days.

Your Eligible Dependents:

  • Your legal spouse
  • Your domestic partner
  • Your children, and the children of your domestic partner, up to age 26
  • Your legally disabled children, with no age limit

Adding Dependents

When you add dependents to your coverage you’ll need to provide their legal name, date of birth and Social Security number, along with supporting documentation — a marriage certificate, birth certificate, adoption papers or tax documents.

If you don’t provide the required information, your dependents may be dropped from coverage.
